El tratamiento endoscópico de la atresia de coanas congénita (ACC) es actualmente el gold standard para esta patología. El abordaje endoscópico fue aplicado en la reparación de las ACC en los años 90’, permitiendo la integración de los principios de la cirugía endoscópica nasosinusal (CENS). Este abordaje debe cumplir los siguientes criterios:
🎯 Realizar una cirugía mínimamente invasiva.
🎯 Recuperar la funcionalidad nasal en una cirugía.
🎯 Minimizar el daño a tejidos de estructuras adyacentes.
🎯 Preservar el correcto desarrollo de las estructuras faciales.
🎯 Minimizar el dolor y disconfort postoperatorio.
🎯 Permitir el inicio precoz de la respiración y alimentación mientras se reduce el tiempo de hospitalización.
Independientemente del tipo de abordaje que se utilice, la reestenosis es la principal causa de fallo de tratamiento. Con el objetivo de evitar la reestenosis, se sugiere el uso de colgajos intranasales para cubrir regiones cicatrizales y disminuir la fibrosis.

The endoscopic approach was applied to CCA repair in the 1990s, enabling the integration of CCA surgery with the principles of functional endoscopic sinus surgery (FESS). It should fulfil the following requirements:

🎯 Provide nasal function in a one-stage operation.
🎯 Minimize tissue damage of neighbouring structures
🎯 Preserve the development of facial structures.
🎯 Minimize postoperative pain and discomfort.
🎯 Enable the early start of spontaneous breathing and feeding and to reduce the duration of hospitalization.
Regardless of the access route and repair technique employed, restenosis is the most frequent cause of surgical failure. With the purpose of preventing restenosis and subsequent reoperation, many authors suggest the meticulous preservation of mucosa and use it as vascularized flaps to avoid scarring.
